Field Trip - PLAY STREET MUSEUM HAPPY VALLEY (Deposit)

We are so excited to be hosting your field trip at PSM Happy Valley! Please know that each child will need to have a completed waiver.  You can email parents this link:  https://waiver.smartwaiver.com/w/65710a6f82598/web/

A few days before your field trip, you can email tanyawong@playstreetmuseum.com a list of the kiddos that will be in attendance and we can verify if parents have completed the waiver. We require a waiver for all minors before they enter the museum. 

Details: The payment is nonrefundable, but you may change your field trip day and time if necessary. Please have each child's parent complete a waiver before the field trip.

We have take home activity kits for only $4!  Also, if children would like to make a batch of slime during their visit to Play Street Museum, they can bring $7 with them on the field trip.
