Number of Kiddos attending who are NOT using a Gift of Play Pass or 5 Visit Pass. (Leave this at “0” if your child is using a pass)

Have a play pass? Register your child(ren) for a session and present your pass upon arrival. (Must bring pass with you to use. If pass is forgotten you will be charged admission of $15 per child at the door)

I understand I am holding a spot so reservations I make with PSM are nonrefundable. If I am unable to attend the session I sign up for, I understand I can transfer to a friend. No need to call, just have your friend let us know upon check-in.
